Why weren't the Columbine (and VA Tech, etc) shooters stopped sooner?

It's only in Hollywood movies that the tactical teams go running into the building only moments after they arrive. Also, the impression I get is that the teams didn't know how many gunmen were involved. Racing in would have been irresponsible. They were being safe, that's all.
